Prepared Health platform to aid discharges at Holy Redeemer

Holy Redeemer Health System, with nearly 30 sites across Pennsylvania and New Jersey, has expanded its use of a platform from Prepared Health that helps hospitals manage post-acute care patients.

Holy Redeemer will use the platform to connect the hospital to life care centers, home care agencies and community-based providers to collaborate in real-time and eliminate gaps in care that can occur when multiple providers are involved with patient care after discharge.

“The technology supports our commitment to safe transitions in care and open, consistent communication with our referral sources and physician partners,” says Patricia O’Brien, senior vice president and chief administrative officer at Holy Redeemer Home Care & Hospice. “This technology allows our team to refer and accept patients for care, update our care partners on patient status, and work collaboratively to prevent unnecessary ER visits and hospital admissions.”

Holy Redeemer further will have access to digital nursing software that analyzes patient data and uses artificial intelligence to identify and recommend evidence-based interventions.

Other new contracts announced this week include:

• Houston Methodist Hospital in Texas will join the TriNetX health research network to improve its ability to offer real-world data for study feasibility, academic collaboration and to increase participation in clinical trials. “The TriNetX platform brings new informatics capabilities that support our vision for the next century by leveraging the innovation of our industry collaborations,” says Edward Jones, president and CEO at the Houston Methodist Research Institute.

• Sturdy Memorial Hospital in Attleboro, Mass., will upgrade its electronic health record system with a new suite of acute, ambulatory and revenue cycle software from Cerner under a seven-year agreement. The vendor’s platform will enable clinicians to share and access data across the entire enterprise and will support documentation being updated in near real-time. The patient portal will integrate with the EHR and enable patients to communicate with providers via secure messaging, and to book and change appointments and view clinical information.

• Anderson Regional Cancer Center in Meridian, Miss., says it is the first facility to treat cancer patients using the RayStation treatment planning system and the TomoTherapy Treatment Delivery System together. The first procedure was administered to a patient with prostate cancer and the center expects to use the combination of technologies to also treat cancers of the breast, lung, head and neck.

• HumanGood, which operates senior living retirement communities across California, Arizona, Nevada, Washington and Idaho, will implement an electronic health record system from Netsmart that will cover all senior living care settings and other facilities. The EHR will support a longitudinal view of an individual’s complete health record across skilled nursing, assisted living and independent living settings. Human Good serves about 10,000 residents in 80 communities.
