Real-time notification helps ManorCare keep tabs on patients

Collaboration with other providers improves outcomes and reduces costs, says Marty Grabijas.


HCR ManorCare is using a technology service to better coordinate with other providers in its communities, with executives of the chain saying that value-based care puts a premium on better transitions and improved communications.

The long-term care chain has selected PatientPing, a software vendor, to provide its application that notifies providers in real time when patients receive care throughout a continuum, better enabling healthcare organizations to collaborate and know what care has been delivered to common patients.

HCR ManorCare operates a network of nearly 500 skilled nursing and rehabilitation services, memory care centers, assisted living facilities, outpatient rehabilitation clinics, and hospice and home health agencies.

To succeed in accountable care environment, providers must collaborate more closely on patients being treated by providers throughout the continuum. ManorCare adopted the PatientPing technology because it wants to reduce unnecessary hospital readmissions, improve outcomes and drive savings for hospitals that face reimbursement penalties from insurers because of unnecessary readmissions, its executives say.

To achieve that ManorCare places nurses in hospitals to write discharge plans; if a patient returns to the hospital, those nurses can refer the patients to a nursing home, if possible, reducing readmission rates.

“If the patient can be stabilized, we can avoid hospitalization and get the patient into a skilled nursing environment,” says Marty Grabijas, vice president of sales and marketing at HCR ManorCare.



“With PatientPing, we’re able to gain valuable patient insights that help providers to reduce hospital readmissions and ease our patients’ transitions from one care setting to the next,” says Grabijas, and his organization also has identified business benefits because patients appreciate better care coordination.
