McKesson Corporation is working with Navigating Cancer, a vendor that offers a patient relationship platform for community-based oncologists.
The platform supports workflow tools to help oncologists provide integrated personalized care while also meeting value-based care requirements that include patient care management tools, remote monitoring to capture patient outcomes and a nurse triage tool that includes symptom management pathways.

The platform further supports notification of patient-reported outcomes and the ability to triage and provide care in real-time. Patients have access to their health information and are able to report side effects before they escalate, receive medication reminders and are able to have a more proactive role in care.
McKesson brings its oncology-specific electronic health record, known as iKnowMed, as well as supporting education programs and the Merit-based Incentive Payment program, known as MIPS.
Navigating Cancer’s patient relationship management platform is designed to increase access to care and improve care coordination to improve outcomes and reduce emergency department visits, says Gena Cook, founder and president of Navigating Cancer.
“We have seen this at Rocky Mountain Cancer Centers, Virginia Oncology Associates, and Minnesota Oncology,” Cook adds.
